Tyoga CareNet Sponsorship of Health Plans

Tyoga CareNet evaluates payors based on the needs of the employer and provider communities, as a whole. Tyoga CareNet seeks out and sponsors the introduction of fully-insured and self-funded health plans. Working closely with employers and payors allows us to create market sensitive fee strategies and balance the needs of health plans and providers, create efficient approaches to utilization management, consolidate claim payment functions, and provide access to utilization data.

How To Become a Member of the Network

A provider must apply to the network in order to become a Member. Currently, only physicians and hospitals are being sought for participation. Other types of providers will be included on an exception basis. Detailed instructions, a contract, and an application form can be downloaded as indicated below. Updated 2005 Fee Schedules for Standard Fee Options

Providers of professional services (such as Physicians, CRNAs, etc.) that want to obtain a copy of the updated 2005 RBRVS fee schedules that relate to the "standard" fee options described in the provider contract will want to download the following file. The file contains an Excel spreadsheet that includes a complete fee schedule for each of the standard fee options, along with a helpful fee lookup function. Be aware, the file is large and will entail a lengthy download if you do not have a high speed internet connection.

Changes in Your Business

Each provider that wishes to become a member of Tyoga CareNet must complete an application that includes the required information for appointment to the network and maintenance of your relationship with the network. This information guides our efforts to oversee quality, your inclusion in provider directories, the pricing of your services, and the payment of claims. It is vital that this information is accurate. In order to maintain accuracy, it is equally vital that you notify Tyoga Carenet of changes in your practice or business that relate to these issues. Consequently, the Network must be notified of the following changes:

• Physical address, payment address, etc.
• Telephone and/or fax number.
• Tax identification number.
• Hospital privileges.
• Status of medical license.
• Professional liability coverage.

For your protection, no changes will be made via a telephone call. All notifications must be confirmed in writing. Please provide written notification of the change with the appropriate signature and mail or fax to the location provided in the "Contact Us" section of this site.
